Overview

The Child Health and Illness Profile (CHIP) is a family of self-report and parent report questionnaires designed to assess the health and well-being of children (Child Edition – ages 6-11) and adolescents (Adolescent Edition – ages 11-17). Domains of health obtained from the CHIP reflect the complexity of children's health. These Domains describe aspects of health and influences on health that can be modified by health care systems, including school health centers, and health promotion interventions.
 
The CHIP instruments were conceptually derived and empirically refined to demonstrate adequate-to-excellent levels of reliability and validity.
